Roles and Responsibilities:
- Review new admitted and readmitted residents into the facility;
- Read through Plan of Care Summary: this section is to communicate to providers any
additional clinical information which may be needed;
- Auditing medical diagnoses entered in the PCC;
- Read Resident’s progress notes written by Nursing department and other discipline in
order to get an idea how the resident is situated in the facility;
- Audit physician’s orders entered: Ensure that all medications have medication name,
dose, frequency, route and Indication;
- Audit orders entered by other disciplinary team; Ensure that all orders have Correct
instructions and indications. This includes: diet orders, out of bed orders, wound care
orders, etc.;
- Ensure that all ADLs have instructions;
- Ensure ALL ADMISSION FORMS are entered and completed and information stated is
congruent to resident’s condition;
- Care Planning: Enter all necessary care plans pertinent to resident’s active problems and
Medical Diagnoses;
- Place care plans for Medications, Allergies, Activities of Daily Living (ADLs), skin
impairment.
Job Requirements:
- Excellent communication skills, both verbal and written;
- Must possess a valid and active USRN license;
- With minimum 2 years of clinical or skilled nursing facility experience preferred;
- Diploma/ Degree in Nursing or related field;
- Must have a high-speed internet connection;
- Can work in a fast-paced environment;
- Must have outgoing and team-oriented personality;
- Strong critical thinking and customer service skills.

Salary: Php55,000.00 - Php77,000.00 per month
